Research Privacy Notice
This survey supports product research about appointment preparation, visit understanding, and care-plan follow-through.
What the survey collects
Responses may include selected answers about healthcare-management experiences, use of digital tools, product preferences, and optional written feedback. Technical anti-spam information, such as completion time and a verification token when enabled, may also be processed.
What we do not request
We do not ask for names, contact information, dates of birth, exact locations, medical-record numbers, specific diagnoses, or other medical-record identifiers. Please do not include identifying information in optional written responses.
How responses are used
Responses are reviewed in aggregate to evaluate product needs and guide product decisions. The survey does not provide medical advice.
Optional synthetic demo access
After a successful submission, the survey may request a one-time demo code from the separate Elestera demonstration service. The request does not include your survey answers or an Airtable record identifier. Codes expire after 30 minutes and are deleted after expiration or redemption cleanup.
Demo learning and AI summaries
The demonstration uses fictional patient information only. Elestera may record limited access, reliability, and structured interaction events to evaluate the experience. Generated AI summaries are not retained by Elestera, and demo activity is not connected to individual survey responses.
Service providers
Netlify hosts the survey and demonstration and processes submissions and temporary access records. Airtable stores survey responses. When enabled, OpenAI processes only the fictional sample document used to generate a demonstration summary. These services act as technology providers supporting this research workflow.
Voluntary participation
Participation is voluntary. You may leave the survey before submitting. This notice does not promise legal anonymity or state that the survey is covered by HIPAA.
